Insurance Certificates

Request a Certificate

For General Liability (GL)  or Worker’s Compensation (WC): The Insured must call our office to request a certificate due to privacy laws.

Please be prepared with the following information :

• Full name of insured including dba

• Name of certificate holder (name of person requesting the certificate)

• Address of certificate holder

• Fax number

Please note on Worker’s Compensation this can take up to 48 hours. In some instances the Insurance Company issues their certificates and then they will fax or mail the certificate directly to the certificate holder.
